Enroll online

We make it easy to enroll online with a 6-step application. Once you submit your completed online application, you will get a confirmation number and we will quickly start processing your enrollment.

If you have recently qualified for Medicare coverage by turning 65 or experience a circumstance that would allow you to enroll outside of the Annual Election Period, you can enroll in one of our Medicare Advantage plans.

Your health needs may change and you might need additional coverage. If you are already enrolled in a Medicare Advantage plan but need to add or remove coverage options, you can sign up for a new plan during the Annual Enrollment Period (AEP). It occurs every year from October 15 to December 7.

Eye exams are covered in-network for Community HealthFirst members. Vision coverage is included in all plans.
