- Our licensed Medicare experts are available to meet with you at your local clinic or another convenient location to help you enroll, discuss your current plan, your coverage needs, and answer any questions you might have about Medicare. Medicare beneficiaries may enroll in Community Health First Medicare Advantage Plans through the CMS Medicare Online Enrollment Center located at http://www.medicare.gov.
*Please note that MA Special Needs Plan 014 (HMO SNP) does not participate in online enrollment.
