Health Assessment

A health assessment (also known as a health risk assessment, or HRA) is a short survey that asks about your health history. Completing it helps us get to know you and better to understand your health needs. That way, we can connect you with services, programs, and resources that better support you and your care.

It should only take a few minutes to complete.  

How do I take a health assessment?

If you are a new CHPW Medicare Member, our CHPW team will reach out by text, email, phone, or mail to complete your annual health assessment. We ask all new members to complete a survey within 90 days, and then we reach out once a year after that.

How can a health assessment help you?


Your health assessment helps us better understand your needs so we can support you better. By completing it, we can:

  • Connect you to services, programs, and resources that fit your health needs
  • Help you access the right care for you
  • Identify ways to improve your overall health and well-being

Your information is kept private and secure. You only need to answer the questions you are comfortable with. The survey will only be used to improve your care and connect you to available resources. It does not affect your health coverage or your eligibility.

We want to help you feel your best by getting you access to resources to help meet your health care needs.

What is included in the health assessment?

Our health assessment generally asks about:

  • Basic information: Age, gender, and race
  • Lifestyle: Exercise, diet, tobacco use, and alcohol use
  • Emotional health: Depression, anxiety, life events, and other stressors
  • Physical health: Weight, blood pressure, cholesterol levels, pregnancy, and other issues
  • Current and previous health conditions
  • Resource and support needs: Food, housing, transportation, employment support, and more

Who needs to take a health assessment?

All CHPW Medicare members will be asked to complete a health assessment yearly, and new members will be asked to complete it within 90 days.

How do I take a health assessment?

You can complete it online, over the phone, or through the mail. We’re required to offer this survey, but you only share what you’re comfortable with.

Completing it helps us know how to support you—whether that’s finding care, connecting with specialists, or linking you to community resources like food, housing, and transportation.

  1. You’ll need your CHPW Medicare Member ID
  2. Select Medicare as your line of business
  3. Only share what you are comfortable with

Do I have to fill out the Health Assessment?

No, but completing it helps us provide better care and services tailored to you.

How long does it take?

Most members finish in about 15-20 minutes.

Is my information kept private?

Yes. Your responses are confidential and used only to improve your care and connect you with available resources.

Will this affect my benefits or coverage?

No. Your survey answers do not impact on your benefits or eligibility.

What happens after I complete my Health Assessment?

Your responses help us identify services, care coordination, or programs that may be a good fit for you. You will likely hear from a CHPW care manager or receive information about helpful resources.

Do I need to prepare for anything before taking the Health Assessment?

Not at all! Just answer the questions based on your current health and lifestyle. There are no right or wrong answers.

Can I skip questions I don't want to answer?

Yes. You’re not required to answer every question, though the more complete your responses, the better we can support you.

Can I complete a Health Assessment in another language?

Yes! We offer the Health Assessment in Spanish.

Can someone help me fill it out?

Absolutely. You can complete the Health Assessment with help from a care coordinator, a trusted family member, or over the phone with one of our team members.

What if I already completed a Health Assessment this year?

If you have already completed one this year, you don’t need to do it again. If you’re unsure, give us a call at 1-800-942-0247 (TTY: 711) 8:00 a.m. to 8:00 p.m., 7 days a week, and we can check for you.

Do I need to complete the Health Assessment if I’m healthy?

Yes! Even if you feel great, your responses help us make sure you continue to get the right support and preventive care.

Can I update my answers later?

If your health changes, you can always reach out to us. We may offer another Health Assessment or connect you with a care coordinator to update your information.
