When you enroll in a Community HealthFirst™ Medicare Advantage plan, you will be sent an ID card.
You will need to show the Community Health Plan of Washington ID card and your state-issued Services card each time you get medical care. That includes medical visits, specialist visits, mental health visits, hospital visits, and pharmacy prescriptions.
If you need a new ID card sent to you, please call Customer Service at 1-800-942-0247 (TTY Relay: Dial 711), 8 a.m. to 8 p.m., seven days a week.
